Type O Negative: Everyone I Love is Dead.
Actually, I think Johnny Cash's cover of "Hurt" makes me sadder, but that might just be because it was Cash's last single. As far as other sad songs go....
Nirvana "All Apologies"
Pink Floyd "Wish You Were Here"
The Tragically Hip "Ahead By A Century"
The Tragically Hip "Fiddler's Green"
Crash Test Dummies "Superman Song"
Beck "Lost Cause"
Meh.
First track on the Eels' Electro-Shock Blues(actually, half the songs on the album will break you in two), Elizabeth on the bathroom floor, saddest song ever. Also, Dead of Winter off the same album.

Without You- Harry Nilsson
Mary Jane- Alanis (argh!)
