i dont know if it's medically possible to never sleep, but i'd say the majority of the time he thinks he's sleeping he's tyler.

he has insomnia. this means he doesnt sleep. but after meeting marla tyler forms and everytime he sleeps tyler is out and about.he thinks he's sleeping but not really. his body feels that it is getting sleep because thats wat he think

i disagree about how tyler shows up right after meeting marla, this is something that has been bottling and accumulating. it really just shows how fucked up the narrator is. he has insomnia and cant sleep so essentially he just becomes a different person for that time and is completely oblivious to it.

I had to be pikced up by an ambulancetoday. I was at the er adn my mother had to pick me up fromintensive care. i passed out on soneone 's lawn. i dont know who's. my lilil sysister had to get me from th er.
